For security reasons, the following procedures must be followed:
We require a fax with an official purchase order on a company letterhead signed by the credit card holder, showing total amount to be charged to the card. The following details must also be included:
- Credit card number
- Expiry date
- Exact name on card
- Faxed photocopy of front and back of card showing signature of cardholder.
- Credit card statement address
